If we take a fantasy setting and all of what usually comes with it, I believe a cloud city would still be the most defensible when compared to an underground or underwater one.
Since a cloud city already implies strong magic involved (And as a result strong magic defenses), the only threat you'd really have to worry about is some sort of army of dragons, but that's really unlikely given that dragons are traditionally solitary. Flying demon armies could still be a problem, but the best thing about a cloud city is you could probably move it if a certain location becomes problematic.
An underground city suffers the same problems as a topside one. One can still invade it with a regular army, etc. Not to mention underground settings usually have even more dangerous creatures living there.
An underwater is a little better since you're more or less protected from "air breathers" and presumably you have strong magic going on here too. You still have to worry about underwater races and creatures that generally don't want you there and if your protective air bubble ever gets disrupted, your citizens are fucked.

Really, though. I hate seeing birds in cages. More than I hate seeing humans or other animals in cages. it just strikes me as...in a very primal sense...wrong.
